Below is a list of REQUIRED paperwork needed to register your child for SAA-SCU. PLEASE bring THREE copies of ALL FORMS to Tryouts.

1. YSSL (Boys) REGISTRATION FORM

2. IYSA EMERGENCY WAIVER

3. Copy of INSURANCE CARD: Current insurance card, front and back.

4. BIRTH CERTIFICATE or PASSPORT:  ONLY IF NEW TO TRAVEL/CLUB SOCCER ~Please note: If your child is NEW to travel/club soccer, the following information is REQUIRED BY THE YSSL. WE CAN NOT REGISTER YOUR CHILD WITHOUT THIS INFORMATION.
